• Your name and role (e.g., parent, provider, district representative, employer) 
• The individual’s name and age (if applicable) 
• A brief description of the services you are seeking 
• A phone number for follow-up 

Someone from our team will be in touch promptly. 

At this time, we do not bill private insurance. We do offer the following options: 

• Services provided through Child Development Services (CDS) 
• Acceptance of CareCredit 
• Potential use of HSA funds (please check with your insurance carrier or HSA administrator) 

As our outpatient services continue to grow, additional cash-based service models will be offered.

• Ergonomic Evaluations for Businesses In-person services throughout Aroostook County, Maine 
• Remote ergonomic evaluations for businesses statewide
